COPHYLLIN SYRUP

The active ingredient of Couphyllin is Etofylline & Theophylline 46.5mg/12.73mg.

Theophylline & Etofylline act as potent bronchodilators by direct relaxation of the bronchial smooth muscle. The action of Theophylline leads to smooth muscle relaxation by inhibition of phosphodiesterase. It also causes relaxation of pulmonary blood vessels, coronary vasodilation, cardiac and cerebral stimulation and diuresis.

In vitro Theophylline has been shown to react synergistically with beta 2 which increases intracellular cyclic AMP through the stimulation of adenyl cyclase.

Prophylaxis and relief of acute and chronic asthma, bronchitis, emphysema and other obstructive airway diseases.

Therapeutic serum levels associated with optimal like hood for benefit and minimal risk of toxicity are between 5mcg/ml and 20mcg/ml. There is great variation from patient to patient in dosage needed to achieve the therapeutic blood level because of variable rates of theophylline elimination. The dosage therefore needs to be individualized.

Initial starting dosage in children should be 16mg/kg/day, which could be gradually increased when required. The daily dosage should be divided and given at 6-8 hourly intervals, preferably after food.

The following is the rough guideline for the dosage based on the age and body weight and should not be exceeded without serum Theophylline levels estimation.

Below 9 years             :           24mg/kg/day

9-12 years                   :           20mg/kg/day

12-16 years                 :           18mg/kg/day

Above 16 years           :           13mg/kg/day

Adult                           :           1 ½ teaspoonful orally every 8 hourly

preferably after food or as directed by the physician.
